We want to make it easy for you to send your handpieces to us. You can request a mailer package by filling out our feedback form making a note that you would like a Handpiece Mailer Package. You may also follow the steps below and print your own forms.
Step 1
Fill Out the Order
Download the Handpiece Repair Ticket.
Complete the Handpiece Repair Ticket on-line or print and then complete.
Step 2
Prepare Shipping Label
Print the Prepaid Mailing Label. Affix label to suit mailing box. If you need a box, please request the Handpiece Mailer Package.
Step 3
That’s it!
Complete a Postal Service Form 3811 if you would like a Return Receipt.
